What Is a LOCUS Assessment?

The Level of Care Utilization System (LOCUS) is a structured clinical tool used to determine the most appropriate level of mental health care based on symptoms, functioning, supports, and treatment needs.

At ThriveLine, LOCUS assessments are comprehensive, trauma-informed, and clinically grounded. Each evaluation includes:

  • Optional narrative psychiatric reporting when requested
LOCUS assessments can help determine whether someone would benefit from outpatient therapy, intensive outpatient (IOP), partial hospitalization (PHP), residential treatment, inpatient stabilization, or more structured supports.

All LOCUS assessments provided by ThriveLine are strictly clinical and therapeutic in nature. They are not legal, forensic, disability, custody, or court-related evaluations.
ThriveLine does not provide:
  • Forensic or third-party evaluative opinions
  • IMEs (Independent Medical Examinations)
  • Fitness-for-duty or disability determinations
  • Custody evaluations
  • Court testimony or documentation for legal disputes

LOCUS Assessment Options

(All services listed below are cash pay and intended for adults age 18+.)

LOCUS Assessment – Standard

  • Duration: 90 minutes
  • Fee: $800

A comprehensive clinical evaluation including review of records, a full clinical interview, LOCUS scoring, and rationale for level-of-care determination. Suitable for most agency or care team
requirements.

LOCUS Assessment – Extended / Complex

  • Duration: 120 minutes
  • Fee: $1,000

An expanded evaluation for individuals with complex presentations, co-occurring conditions, or extensive treatment histories. Allows for more thorough interviewing and detailed review of records.

LOCUS Assessment with Full Narrative Psychiatric Report

  • Duration: 90–120 minutes
  • Fee: $1,200

A comprehensive psychiatric evaluation integrating LOCUS scoring into a full narrative report, including history, diagnostic impressions, and treatment recommendations. Turnaround time: Up to 7 business days.

Rush Processing Add-On

  • Fee: +$300
Provides priority scheduling and delivery of LOCUS results within one business day. Must be paired with a LOCUS assessment.

    program

How a LOCUS Assessment Works at ThriveLine

A thorough psychiatric interview assessing all LOCUS domains (risk, functioning, medical/substance comorbidity, environmental stress, support system, engagement, etc.).
You receive a clear explanation of your LOCUS results and their clinical meaning.

If selected, a detailed psychiatric report is provided within seven business days.

ThriveLine collaborates with community partners to support accurate placement and continuity of care.

Services include:
